The COVID-19 Pooled Testing Consent Form is now available on your portal! Please fill out the form as soon as possible.

You may access the form(s) by clicking the link in the yellow ribbon at the top of your screen. Forms are due by noon on Thursday, November 5 for pool 1 or Friday, November 6 for pool 2.
