• WHAT IS MAGNUS?

  1. It’s a digital, student health management system.
  2. It collects and tracks forms so that we can keep your children safe and healthy.
  3. Access it via the Magnus Health button on the Resources page on your Parent Portal (the LMS).

• WHAT ACTION DO I NEED TO TAKE, AND WHEN?

  1. Schedule your child's annual physical as soon as possible.
  2. All physicals for the 2023–24 school year must occur before August 15, 2023.
  3. All requirements must be submitted by the August 15, 2023 deadline for review and approval. 

  *If you are currently living abroad and will not arrive before the deadline, please upload all of your current international records and submit updated forms once you have arrived in the US.

• WHAT HAPPENS IF I MISS THE DEADLINE?

  Please make every effort to submit all required forms before the August 15, 2023 deadline. If you are in need of an extension you must contact magnus@fasny.org.
  
  If you miss the deadline please understand your child will not be able to do any of the following until all Magnus requirements are complete:

  1. Participate in pre-season athletics, rehearsals, or any other school-run activity.
  2. Participate in any field trips.
  3. Attend school.

  If your child wishes to take part in any school-related activity this summer, all Magnus requirements must be completed prior to their participation.

• HOW DO I SUBMIT MY CHILD'S MEDICAL RECORDS?

  Please submit the required forms by uploading them into your Magnus Health portal. Log into MySchoolApp, and select the Magnus button on the resource board. More information about options for uploading your forms can be found here!

  
  Remember that all pages must be uploaded to constitute a complete submission.

   

  Please do not mail any documents to the School, as our nurses cannot process them. For the sake of confidentiality, any forms sent to the School will be destroyed.

• CAN I DELIVER A COPY OF MY CHILD'S FORM TO THE SCHOOL NURSE?

  Parents in our community are responsible for ensuring that their children's health information is accurate. Nurses are responsible for our student's daily health and safety and cannot be responsible to upload your child's health forms. For the sake of confidentiality, any forms sent to the school will be destroyed. Parents/guardians must complete the uploading themselves (see “How Do I Contact Tech Support” below if you need assistance).

• MY CHILD IS ON A SPORTS TEAM, DO I NEED TO DO ANYTHING SPECIAL?

  It is important to note that students who participate in fall sports need to make sure to get all requirements completed as soon as possible. Students can not begin preseason practices without all necessary forms completed. 

  Please do not wait until the last minute!

• WHAT IS THE MAGNUS HEALTH MOBILE APP?

  A simple and easy way to upload your requirements! Magnus Health has developed the Magnus Health mobile app, which allows parents to submit their student's forms from any 'smart' device. Photos can be taken directly from your device and uploaded to your portal and you can fill out important health data all while using this new app.

  Getting Setup on the Magnus Health Mobile App:

  1. 1. Navigate to the Magnus Health portal via the Resource Board on the LMS.

  1. 2. When you are logged into Magnus Health, hover over your name in the upper left corner of the screen.

  1. 3. Choose "Change Credentials."

  1. 4. Create a Username & Password that only you know and will remember. These will be the credentials that you will use for the mobile app login only. This will not change how you access the Magnus Health portal through the Resource Board on the LMS.

  1. 5. On your mobile device, download the new mobile app that is named "Magnus Health" from the Apple Store or the Google Play store.

  1. 6. Log into the new Magnus Health mobile app from your phone using your new username & password.

• WHAT IS CAMSCANNER?

  With CamScanner, any paper documents can be digitized and saved right away with your mobile phone. It's just like taking a photo but it will save the image as a PDF. No need to pay for the full version—the free version does everything you need!

• WHAT ARE MY CHILD'S IMMUNIZATION & VACCINATION REQUIREMENTS?

  We advise parents to download the NYS Requirements for vaccinations here (updated annually). Immunizations apply to ALL students. 

  
  Children in a pre-kindergarten setting should be age-appropriately immunized.
  
  Intervals between doses of vaccine should be in accordance
  with the recommended immunization schedule for persons 0 through 18 years of age.
  
  Doses received before the minimum age or intervals are not valid and do not count toward the number of doses.
  
  *If you are currently living abroad and will not arrive before the deadline, please upload all of your current international records and submit updated forms once you have arrived in the US. 

   

• HOW DOES FASNY USE MY CHILD'S MEDICAL INFORMATION?

  1. All requirements are reviewed by FASNY staff for accuracy.
  2. Pertinent information is shared ONLY with those who need it when they need it.
