Ski Club Form 2016-2017

Dear Parent:

We are pleased to announce the reopening of the ski club now open to students from 5th to 12th grade. Three ski trips to Hunter Mountain, NY, have been scheduled this season for December 4, 2016, January 8 and March 4, 2017. (Rain date: March 12).

Prices for Grades 5 to 9: All 3 trips : $375 without equipment – $450 with equipment / 1 trip : $140 without equipment - $165 with equipment.

Prices for Grades 10 to 12: All 3 trips : $230 without equipment – $305 with equipment / 1 trip : $90 without equipment - $115 with equipment.

Prices for Adults: 1 trip: $70 without equipment - $95 with equipment.

I do hereby give permission for my child to participate in the ski trips to Hunter Mountain organized by the French-American School of New York between December 8, 2016 and March 12, 2017. I give my child permission to be transported to and from HUNTER MOUNTAIN by coach bus.


I also authorize the personnel of the French-American School of New York to take my child for emergency care in the event neither parents nor guardian can be contacted.

I understand that in order to attend the ski/snowboarding trip, me and or my child must be covered by a family insurance plan. I also understand that, in case of injury during the trip, our family insurance plan will act as the primary insurer. Below is the necessary insurance information:

In consideration for being permitted to participate in the identified activity, I, for myself and for my personal representative, assigns, heirs, and next of kin, do: 1. ACKNOWLEDGE that I understand the nature of skiing activities that I am physically and mentally qualified to participate in such activities. 2. ACKNOWLEDGE that it is my own responsibility to be appropriately clothed and that my equipment has been checked for safety within the past 12 months. 3. ACKNOWLEDGE that cross-country skiing and other outside winter sports involve risks and the danger of serious bodily injury, including permanent disability, paralysis, and death. I understand that the volunteer trip leaders and the French-American School of New York are not responsible for screening participants for ability, proper attire, or equipment. 4. ACKNOWLEDGE that I have read and will adhere to the rules of the mountain as stated in the Hunter Mountain Responsibility Code: 1. Always stay in control, and be able to stop or avoid other people or objects. 2. People ahead of you have the right of way. It is your responsibility to avoid them. 3. You must not stop where you obstruct a trail, or are not visible from above. 4. Whenever starting downhill or merging into a trail, look uphill and yield to others. 5. Always use devices to help prevent runaway equipment. 6. Observe all posted signs and warnings. Keep off closed trails and out of closed areas. 7.
