The New York State Department of Health requires all children to be immunized against certain communicable diseases. Proof of a child’s immunization is mandatory before being allowed to enter school. Failure to provide such proof can result in that child’s termination from school (see Immunization Requirements).

The New York State Education Law also mandates that all new students, as well as children entering Grades K, 2, 4, 7, and 10, furnish proof of a recent physical examination by a U.S. physician (see  Physical Examination Certificate).This report (and the Health History Review) is also required for all Upper School students participating in a sport team within one year of the first day of practice.

The State of New York Department of Health has recently changed the immunization requirements for school entry into the 6^th grade.  To read more about this requirement, click on Tdap Immunization.

Children requiring the administration of medication during school hours (including emergency medications and OTC medications) need to have a current written doctor’s order, as well as a parent’s written permission to give such medication (see Medication Administration). It is also important that the parent provide the nurse with the medication in question. Failure to comply with these policies makes it impossible for your child to receive the medications they need at the appropriate time.

Families that have children with special health needs must inform the nurse. Children with allergies or asthma must fill out the Allergy & Asthma Forms (see Allergy or  Asthma Information) each year and return them to the nurse’s office as soon as possible.

**For Upper School Students**

According to N.Y. State Education Department guidelines the following forms are required in the nurses’ office prior to the first practice session for all Upper School students interested in participating in FASNY sports:

1) A current physical exam done by a U.S. physician within 12 months (see Physical Examination Certificate)

2) A health history review completed by a parent or guardian within 30 days (see Health History Review)

Students missing one or both forms will not be allowed to participate until these requirements are met. Return forms to the Upper School nurse so the child may be cleared to participate in the sport.
